Compensation: $46,000 – $54,000 annually (DOE)

Healthy At Home Caregivers is growing, and we’re hiring a Care Coordinator to support families, caregivers, and our office team in delivering non-medical in-home care.
What You’ll Do
Care Coordination & Assessments
Manage referrals and coordinate client assessments
Schedule and complete limited in-person assessments (designated field day — typically Wednesdays)
Communicate clearly and compassionately with families and caregivers
Ensure assessment documentation is accurate, complete, and submitted on time
Coordinate clean handoffs to scheduling so care starts smoothly
Service Tracking & Follow-Up
Track service starting dates, renewals, and approved hours
Follow up proactively so no services or hours are delayed or missed
Support alignment between care delivery, documentation, and office workflows
Administrative & Office Support
Maintain accurate client and caregiver records
Handle documentation, forms, and follow-ups with attention to detail
Support office workflows during busy periods
Ensure actions are documented and easy for others to follow
Team & Growth Support
Support leadership with coordination and documentation
Identify workflow gaps and improvement opportunities
Be a reliable, calm presence in a fast-moving office

Requirements
Requirements & Compliance
Ability to pass a Live Scan (background check)
Ability to complete a TB screening
Valid driver’s license required
Comfortable working in the office and attending limited field visits
Use of company vehicle during work assignments
