Social Worker - Homeless Outreach Team (Oceanside, North San Diego County)


compensation: $18.00 - $21.00 per hour DOE

OVERVIEW: Interfaith Community Services was founded in 1979 by a handful of diverse faith communities to address the needs of low-income, homeless and under-served people in North San Diego County. Interfaith offers programs through North San Diego County, from the coastal City of Oceanside, to the inland communities south and east of our Escondido headquarters. With more than 300 member faith centers and 130 dedicated staff, Interfaith serviced more than 16,000 unique individuals last year alone.

WHAT YOU WILL DO: This position works in collaboration with the Oceanside Police HOT team to address the issue of homelessness. The HOT team is a unique police outreach effort that connects homeless individuals with needed services in a focused effort to end homelessness.

• Conducts outreach and engagement with individuals experiencing homelessness within the City of Oceanside
• Develops trusting relationships with individuals and families experiencing homelessness
• Bridges clients to appropriate crisis resources to include crisis houses, domestic violence and sexual assault services, human trafficking safe havens, and local hospitals
• Facilitates acceptance of placement or other services that support and stabilize individuals moving from street to service
• Utilizes motivational interviewing techniques to understand, assess, and shift the individual’s stage of change
• Collaborates with client’s family members, friends and existing service providers
• Stabilizes individuals by linking them to appropriate resources such as housing, medical, mental health, addiction and recovery services, food, clothing, transportation, employment, Social Security Disability, mainstream benefits and other financial resources based on the client’s individualized plan
• Facilitates and maintains community relationships to create referral pathways for program participants into Interfaith’s programs
• Maintains appropriate documentation and data collection of clients being served
• Engages with the ARS Bridge Housing Network which consists of homeless service and shelter providers
• Researches and Implements best practices on street outreach and assist participants in acquiring resources for acquiring housing
• Develops and manages a comprehensive outreach/ working relationship with all stakeholders, elected officials and staff, commercial and private community activists, and clients
• Participates in case management meetings, staff meetings and trainings
• Performs additional duties as assigned

• Master’s Degree in Social Work, Counseling, Marriage Family Therapy, Public Health or related field required
• 1+ years of experience working in a Social Services, Nonprofit or related setting required
• Experience working with severely mentally ill, medically fragile, and homeless populations preferred
• Master’s Degree in Social Work (MSW) preferred

• Ability to establish and maintain professional boundaries with clients
• Demonstrated strong crisis management skills
• Demonstrated strong working knowledge of resources in North County San Diego
• Must have reliable transportation and possess and maintain a valid California driver’s license, including proof of personal vehicle insurance coverage and insurability under the agency’s insurance carrier’s standards
• Must successfully pass a criminal background and exclusion/disbarment check
• Must successfully pass a pre-employment physical, tuberculosis test, and drug screen
