Late 1950s to mid 1960s era General Electric model F15A2 2 speed fan. These were advertised as an All Purpose fan in that they could be used on a table top, sitting on the floor and tilted up or hung on a wall and angled downwards. There are slots on the back side of the base to slide over screws to hang on the wall or it sits on rubber feet when using it normally.

Works great (on both speeds) and looks great in 2 tone white and teal.

About 14" tall and wide.
