This the Honeywell model RTH111B1001. The controls are: Fan, On or Auto and Cool, Off, Heat. Its not for use with a heat pump. You set the furnace or air conditioner and the temperature and that's about it. It worked for us quite well till it was removed.
It measures:
4 5/8 inches high
3 inches wide
and
1 inch deep
Wiring diagram is on the inside of the back. There is a manual at Manualslib (https://www.manualslib.com/manual/770853/Honeywell-Rth111.html). I can also email it to you. Installation is your responsibility. I can only guarantee that is was working when the technician uninstalled it.
It requires two AAA batteries that I will include.
